Primary Care

Worcester RISE for Health provides comprehensive primary care services to individuals of all ages, from newborns to the elderly. These services focus on preventive care, such as routine check-ups, vaccinations, and health education. The program also offers chronic disease management , and provides acute care for sudden illnesses or injuries.

Maternal and Child Health

The Maternal and Child Health program at Worcester RISE for Health provides comprehensive care to expectant mothers and their children. We offer prenatal care, newborn care, and post-delivery care. RISE’s family medicine providers work hand-in-hand with RISE’s Community Health Workers, who are specialized in Maternal and Child health.

Using a trauma-informed care approach, we help mothers navigate childbirth and early motherhood by connecting them with essential resources like transportation, car seats, newborn assessments, family planning, and support for stressors of parenthood.

Our Community Health Workers are also trained Doulas, so they extend direct support to our patients in pregnancy and in labor. In addition, we are also thrilled to connect community members to our network of highly skilled and trained Refugee and Immigrant Community Doulas.

Community Health Workers (CHWs)

At the heart of Worcester RISE for Health is its team of Community Health Workers (CHWs). The CHWs serve as vital connectors between clients and the healthcare system, helping refugees and immigrants navigate complex healthcare services and access essential resources. CHWs, who often share the cultural backgrounds and languages of the people they serve, offer personalized support, addressing social determinants of health and advocating for the needs of the community. By building trust and relationships, CHWs help clients overcome barriers such as language, transportation, and unfamiliarity with the healthcare system.

Behavioral Health

The Behavioral Health program at Worcester RISE for Health addresses the mental health needs of refugees and immigrants by offering culturally competent care. Recognizing the unique trauma and stressors these populations face, the program promotes psychological well-being and supports successful integration into the community.
