Civil Surgeon Program (I-693)
At Worcester RISE for Health, we are proud to offer immigration medical examinations(I-693 Form) through Dr. Olga Valdman, a certified Civil Surgeon authorized by U.S. Citizenship and Immigration Services (USCIS).
If you are applying for a Green Card (adjustment of status to lawful permanent resident), you are required to submit Form I-693: Report of Medical Examination and Vaccination Record. Our team at RISE is here to support you through this process. We provide the medical exam, assist with completing the necessary documentation, and ensure that everything meets USCIS requirements.

The fee at Worcester RISE for Health to fill out the I-693 Form is $200 and involves two appointments.
The first appointment has a cost of $50. You will need to bring your passport, vaccination record, and A number ( if applicable).
On this appointment, you will be informed about the required lab work as well as if there are any vaccines missing.
The second appointment has a cost of $150. You will need to bring proof of new vaccination ( if applicable).
On this appointment, you will have the physical exam done by the Civil Surgeon, and your document will be handed to you in a sealed envelope.
Note: The fee does not include lab exams that are required for us to fill out the form and does include vaccines that you may or may not need.

Our Process and Fees
The fee at Worcester RISE for Health to fill out the I-693 Form is $200 and involves two appointments. The whole process usually takes 2-4 weeks.
The first appointment has a cost of $50. You will need to bring your passport, vaccination record, and A number ( if applicable).
On this appointment, you will be informed of any vaccine requirements, and all necessary laboratory tests will be ordered at that time
The second appointment has a cost of $150. You will need to bring proof of new vaccination ( if applicable).
On this appointment, you will have the physical exam done by the Civil Surgeon, and your document will be handed to you in a sealed envelope.
